To Pay by Credit Card: Please print, complete and fax or mail the printer-friendly order form to the information provided on the form. Please note, if you prefer to pay online, an online order form for credit card purchases will be available on this page prior to the conference.
To Pay by Check: Please print, complete and mail the printer-friendly order form along with your check to the address provided on the form.
Please Note: Binders and CDs will not be shiped till after the conference to ensure that all information, including on-site changes to presentations, will be included.
