*Please note that there will be a dinner workshop limited to hospitals and SNHPA corporate partners on Monday, February 26, 2007 from 6:30-8:30 pm at the conference hotel.  There will be an additional cost of $50 to attend this workshop and registration for the event is required.

**The registration deadline is February 15, 2007 , however space is limited so we urge you to register now.**

 Provide Participant Information (Step 1 of 4)
Click on the Register Now link at the bottom of the page to begin your registration.  All registration and payment forms are run through a secured server to protect your information.

New Feature: To make it easier for conference participants to register for future events or to update information for this event if requested, you will be creating an account while submitting your contact information.  Your e-mail address will be used as your username.  When registering for future conferences you will be able to login to the conference registration and use your stored contact information.

Please Note: Your personal information will never be shared with any other organization/company and only contact information will be saved, no financial data, such as credit card numbers, will be saved during the registration process.

Required fields are indicated with red asterisks (*).  Please check to make sure that your e-mail address is entered correctly.  When the form is complete, click on the Submit and Proceed with Registration button.
 Provide Registration Information (Step 2 of 4)
After submitting your contact information you will be asked to submit some event specific information such as whether you will be attending a pre-conference workshop.  Once you have made your selections, click on the Submit and Continue with Registration button at the bottom of the page.
 Confirm Registration Information (Step 3 of 4)
Once you submit your participant and registration information, we ask that you take a minute to review what you have submitted to ensure accuracy.  If you find any mistakes you can correct them by clicking on the Edit Participant Information or Edit registration Information links on the confirmation page.  Once the information has been reviewed, click on the Proceed to Payment button at the bottom of the page to continue with your registration.

Shortly after submitting your registration information, you will receive an automated e-mail message.  This message will confirm receipt of your registration information and provide additional information and updates.
 Secure Electronic payment (Step 4 of 4)
Upon successfully completing the registration form, you will be linked to a secure payment form to process your appropriate payment amount.   Your registration is not valid until payment is received.

If you would like to pay with a check instead of a credit card, you can still register online, but instead of filling out the credit card processing form during step 3, simply click on the printer-friendly registration form link and print and mail the completed form along with your check to the address provided on the form.

(Please note that by printing the registration form during step 3 instead of from this page, the form will be pre-printed with your registration information, you will only need to fill in the payment information.  If you print the form from the link below, the form will be blank and you will need to fill out all of the registration information.)

The registration fee is refundable up to January 26th.  All requests for a refund prior to January 26th will receive a full refund, minus a $50 administrative fee.  There will be no refunds on registration fees after January 26th although you can send a replacement from your organization or company.  Please notify us if you send a replacement to this conference.

If you pay by credit card, you will receive a separate e-mail confirmation receipt from SkipJack, our credit card processor, when the transaction is approved.  Currently, we accept VISA, MasterCard and American Express.

The pre-conference workshops are half day sessions designed for providers and manufacturers interested in learning the basics of the 340B program.

Note: You cannot register for a pre-conference workshop only.
You must register for the main conference as well.
